Initial Appointments: General Information for All Locations
- Our answering service (877-807-5120) functions to schedule appointments and take messages only. They are not on-site or local, and cannot assist directly with directions to our facilities (they will provide with the website address for directions: www.jfzassoc.com). We encourage our clients to bring their mobile phones with them, as this is the best source for direct, last minute communication on both sides
- We do require 24 notice for any appointment cancellation, as this is a common courtesy. Last minute cancellations waste valuable clinical time that other patients are seeking to utilize
- Please bring your insurance card with you. You have approximately 10 minutes of initial paperwork to complete at your first appointment
