Health and Rehabilitation Psychology
We have notable experience in working with psychological adjustments of patients who are experiencing severe and / or chronic, challenging medical conditions such as diabetes, cardiac rehab, reduced mobility from hip/knee replacement surgeries, stroke-related issues, and chronic pain. Dr. Zagotta works on the rehab unit at Westlake Hospital closely with Physiatrists and the comprehensive rehab therapy team (physical, occupational, speech therapists) in providing rehabilitative treatment. Follow-up for these patients can also occur at the outpatient clinic. Research has shown that addressing psychological needs can improve medical regimen compliance treatment outcomes. Psychological services may be a valuable adjunctive treatment along with traditional medical care toward this goal. In recognizing the importance of psychological services in this area, Medicare has created behavioral health procedure codes that allow psychological services to be billed directly under medical diagnostic codes such as diabetes, CVA, and pain-related diagnoses. Psychological services can be a very effective tool in addressing and limiting obstacles that stand in the way of positive medical outcomes.
Medication monitoring
We have extensive experience working with adults who are using psychotropic medications prescribed by their primary care or internal medicine provider. Patients deemed to benefit from antidepressant or anxiolytic medications can benefit greatly from direct therapy services and ongoing status monitoring by a licensed mental health professional as an adjunct to overall treatment effectiveness. Research would support both antidepressant use and psychotherapy as the most effective combination of treatment for depression. While affecting quality of life issues, anxiety-based and depressive conditions, can often hold comorbidities for other somatic issues and result in increased medical visits. These conditions may also cause complications with compliance to medical treatment regimens, including prescriptive, dietary, and behavioral regimens.
Marriage and Family Therapy
Marital conflict can be a notable stressor affecting quality of life issues as well as complicating medical intervention regimens. Notable stress from marital conflict can be traced to increased prevalence of alcohol or substance abuse as well as impact stress-sensitive conditions such as high blood pressure and other common somatic complaints. Symptom focused marital therapy can typically have positive outcomes. Enhancing basic communication and assertive problem solving skills can prove very beneficial for couples with short- and long term coping difficulties. These focused cognitive-behavioral interventions can minimize symptom areas (e.g., stress, sleep problems, and increased alcohol consumption) that may typically exacerbate comorbid medical conditions.
Pediatric Behavioral Health Services
Diagnostic services for children and adolescents play a particular importance for pediatric and primary care physicians when considering psychotropic medication interventions. Assessment services can provide further diagnostic clarification for emotionally and behaviorally challenged children / adolescents. This diagnostic clarification can include identifying specific emotional and behavioral states that are at clinically significant levels when compared to the population at large, and may warrant specific medication intervention trials. Furthermore, diagnostic services can provide differential clarification of childhood disorders including developmental delays, depression, anxiety, ADHD, and bipolar disorders. Dr. Angela Lew has extensive experience in the field of child & adolescent psychological assessment, including the completion of a psychology postdoctoral fellowship in the Department of Child Psychiatry at the University of Texas Medical Branch, Galveston, TX. In addition to diagnostic services, if medication treatment is utilized, close monitoring of patient response to this intervention, as well as the provision of ongoing behavioral health care is an important component of the overall treatment plan. The effectiveness of the medication titration process can be enhanced by the inclusion of a licensed mental health professional working with a pediatrician or a family care physician.
Many families seek medical assistance for children and adolescents whose presenting concerns may be primarily behaviorally or emotionally based or with these concerns secondary to a medical condition. Behavioral management training for parents as well as cognitive-behavioral therapy to address emotional and behavioral controls for children and adolescents are strong adjuncts to medical treatment planning and can promote positive overall medical regimen compliance and treatment outcomes as well.
